The Three Things That Actually Matter

Botox aftercare is simpler than most clinics make it sound. The three things that actually matter in the first four hours are: stay upright (do not lie down), do not massage or rub the treated areas, and skip intense exercise. Everything else — sleeping on your back, avoiding flying, not bending over — is largely unsupported by clinical data.


Why You Should Stay Upright

Do not lie flat for at least four hours post-treatment. This restriction exists because Botox can theoretically migrate to adjacent muscle groups before it anchors to the target receptor, potentially causing temporary eyelid heaviness. After four hours, sleeping position does not matter.


Skip the Gym for the Day

Skip the gym for the rest of the day. Elevated heart rate and increased blood flow accelerate the spread of Botox before it fully binds. One missed workout is not worth compromising two to three months of results. Resume normal exercise the following morning.


What Else to Avoid in the First 24 Hours

Do not massage or press firmly on the injected areas for 24 hours. This applies to facials, facial massage tools, and CPAP masks. Light skincare — moisturizer, gentle cleanser — is fine. Avoid heat treatments (sauna, hot yoga, steam room) for 24 hours as well.

Makeup is fine immediately after treatment. The injection points are tiny and close quickly. Foundation and concealer can be applied the same day as long as you apply gently without pressing on the treated areas.


What to Watch For

Bruising at injection sites is normal and resolves in 3–7 days. A small firm bump at an injection site that persists past 24 hours is usually a small hematoma — apply a cold compress and it will resolve on its own. Call your provider if you experience significant swelling, vision changes, or drooping that starts after day two.
